Deterministic Tile Composition for Thumbnail, QR, and URL Art

The thumbnail system now treats URL Art as a true thumbnail class. URL Art occupies the thumbnail layer itself and therefore must export exactly the way it appears live.

Canonical layer order

1. Thumbnail imagery or URL Art
2. QR layer
3. Type pill
4. Small URL footer text
5. Optional top text overlays

That layer order must not change. Export must preserve the same order instead of recomposing tiles differently.
